The following documents are required with your written request to bind coverage:
- Completed, signed & dated application
- Copy of quote with limits and any optional coverage’s selected
- Signed Terrorism Form (for Commercial Products only)
For terms provided on a non-admitted basis:
- Surplus Lines Disclosure & Acknowledgement or
- Statement of Diligent Effort
Payment Terms
- Direct Bill: USLI will invoice the insured directly based on the payment plan you choose. DO NOT send SMO payment for any policies that are requested bound as direct bill. Indicate this option by checking “Direct Bill” on page 1 of the quote.
- Agency Bill: Payment required 10 days from inception date of coverage.
An active brokerage agreement with Shelly, Middlebrooks & O'Leary is required in order to request coverage bound. If your agency does not currently have an active agreement in place, the brokerage questionnaire and other required documents can be completed online via our Agent Portal.
All Instant Quotes expire 60 days from the issue date and should be reconfirmed after that time. This proposal is based on the underwriting and rating information provided by you. The coverage and terms being offered may not be the same or as broad as requested. Please review these terms carefully and advise our office if you have any questions.
